Bringing your completed New Patient Packet, photo ID, insurance card(s), and a form of payment will help to expedite your visit. Medicare, Blue Shields, Anthem Blue Cross, TriCare, HealthNet, United Healthcare, Cigna, Aetna. HMO plans: Coastal Communities Physicians Network & Physicians Choice Medical Group (both requires prior authorization and referrals from primary care physician), and others.
